All treatment cycle fees are due prior to the start of the cycle. All outstanding balances must be paid prior to starting any treatment. Payment may be made by cash, check, Master Card, Visa, American Express or Discover. Prices may change without notice.

* Tubal ligation fee includes surgical fees, anesthesia, overnight hospital stay and other charges.
** IUI cycle prices vary depending upon the monitoring requirements. For donor sperm insemination (TDI) cycles additional fees such as sperm sample costs, shipping charges, short-term sperm storage or additional sperm sample processing prior to IUI may apply.
*** These prices do not include any costs charges by outside facilities, such as hospitals, and pharmacies. The anesthesia fee is included in all IVF with egg donation cycles. Medication costs are not included. PGS shipping and handling is not included.
